Educational Benefit Plan

The Educational Benefit Certification form is available through DocRoute. You must login using your single-sign-on credentials, go to Human Resources (left side menu), then click on Educational Benefit. To initiate a new form click on Add New Request.  The form will require the student's Jag Number as well as your supervisor's email address. 

The Educational Benefit Certification form for Affiliates and University Retirees is available below and must be submitted to Human Resources.
